Credentialing Application Login

For Initial Login:  Enter your full email address you provided to the Medical Staff Office in the available space.  Click "First Time Login" which will generate an email to be sent to you.  Login to your email account and retrieve the new email that was sent to your email address.  Click on the link in the email message.  This link will send you to a new screen that will allow you to establish your password.  

Please Note:  When creating your password, do not use any special characters such as !, #, etc. as part of the password.  The signature section of the system will not accept a special character.

Subsequent Logins:   Enter your email address and the password that you established using the initial login or forgot password link.
